Includes a single extra leaf. The top is 29.5" high. Without the extra leaf, it is 54" long and 40" wide. The leaf is 12", so it is 66" x 40" with the leaf. Could not find a maker's name. All wood construction and well made. Has some minor scuffs along one edge of the table from being stored, and a few small chips - see pics, but appears fairly new and in otherwise excellent condition.
